- The unsigned document
- A current government photo ID or acceptable ID plan for the signer
- Room number, facility name, and visitor instructions
- Any required witnesses who are not facility staff unless allowed
- A contact person who can meet me if access is controlled
- Reading glasses, hearing aids, or communication support if needed

- The signer is alert enough to communicate willingness
- The signer is not being pressured by family or staff
- The document can be read aloud if the signer requests it
- The signer can sign, mark, or use another lawful signing method
- The certificate, date, and venue match the appointment
